Any ticket holder can dedicate our Mother’s Day Show you your mom!  Complete the form below and submit a photo of your mom.  You need to follow Kings of Kings International as we post to your social media via ours.Select any of the social media icons to follow.

INSTRUCTIONS
Enter your Mom’s first name only.  
Select your favourite artist or your Mom’s favourite.
Provide permission to use you’re Moms’ name or not
Enter your name & email address so when can let you know when it is posted!
Enter your social media name or handle.  (i.e. @yoursocialmedianame)
Provide your date of birth as you must be over 18 to provide permission
 Select a photo to upload. (Works with Apple iPhone, Android Phone, tablets and computers)
Select what platform to share on.  (*Recommend Facebook as it easy to share your account)
Agree to the terms
Enter the date you submitted the form
Click submit!    (Only one submission per ticket purchased)

 

Privacy:  We do not share your information with any external entities. We will create a post that will be on our social media for people to see your dedication.  We do not sell your information. After the post is created, all data is kept for confirmation of the submission.  You will receive a conformation from the email address by this address. no-reply@kingsogkingsinternal.com Check your spam folder should you not receive the confirmation.

I hereby grant the KINGS OF KINGS INTERNATIONAL(herein "COMPANY"), its directors, officers, employees, agents, and designees (collectively “KINGS OF KINGS INTERNATIONAL”) non-revocable permission to use my image uploaded and likeness (collectively “Mom;'s Picture”). I acknowledge that the COMPANY will own such Images and further grant the KINGS OF KINGS INTERNATIONAL permission to copyright, display, publish, distribute, use, modify, print and reprint such Images in any manner whatsoever related to COMPANY show WONDERFUL VIBRATIONS CONCERT, business, including without limitation, publications, advertisements, brochures, web site images, or other electronic displays and transmissions thereof. I further waive any right to inspect or approve the use of the Image by the COMPANY prior to its use. I forever release and hold the COMPANY harmless from any and all liability arising out of the use of the Images in any manner or media whatsoever, and waive any and all claims and causes of action relating to use of the Images, including without limitation, claims for invasion of privacy rights or publicity.

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name or, if I am less than eighteen years old, that my parent or guardian has signed this release form below. This release is binding on me and my heirs, assignees and personal representatives.
